What You Will Do
The Care Manager, Social Worker is by trade a Licensed Social Worker or Licensed Counselor. This role will have overall responsibility for supporting the behavioral health, mental health and substance abuse needs. The Social Worker Care Manager on the Central Team will have expertise in Behavioral Health Care Management Programs.
Key Responsibilities:

  • Develops and executes member's plan of care including appropriate protocols and care paths for all levels of clinical complexity.
  • Communicate goals and patient status to partners of the member's healthcare team, often acting as patient advocate.
  • Use of appropriate IT platforms to communicate and store information and evaluate outcomes.
  • Systemically work with others in Ascension's Network to ensure we are engaging patients throughout the continuum of care.
  • Participates in the execution of behavioral health strategy.
  • Demonstrates proficiency and support of IT platforms related to care management documentation.
  • Develops community resource expertise.
  • Serves as liaison to medical homes, physician practices and community partners.
  • Provides assessment of member's complementary healthcare needs and develops member plan of care to assure provision of such needs.
  • Identification of opportunities for and provision of elements of health maintenance and population.
  • Management including wellness and activation objectives for both the population and individual member.


Key Requirements of role:

  • Yearly this role will require education specific to cultural competencies and social determinants of health.
  • Yearly this role will review and complete the Behavioral Health Competencies set forth in the National CM Program.


Coordinate comprehensive plan of care for patients.

  • Oversee primary needs of patient, including assessment of client and family/caregivers, and develop comprehensive plan of care.
  • Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.
  • Monitor patient care through assessments, evaluations and/or patient records.
  • Advocate for resources and removal of barriers.
  • Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.


What You Will Need
Licensure / Certification / Registration:

  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Licensure required relevant to state in which work is performed.
  • Counselor preferred. Licensure required relevant to state in which work is performed.
  • Social Worker preferred. Licensure required relevant to state in which work is performed.


Education:

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
