We are seeking a National Director for our Care in the Home team! The ideal candidate would be an RN with 5-10 years of leadership experience in a Home Health/Case Management setting.

What You Will Do
Direct assigned program(s) to meet business objectives.

• Direct program team to achieve goals.
• Define program goals and infrastructure to align with business development strategies and partnerships.
• Direct program through creating program plans, management tools and reporting capabilities.
• Develop program budget and monitor and adjust plan as resource requirements change.
• Communicate and document program quality outcomes and progress metrics.
• Serve as liaison between programs, other internal departments and external partners.
• Provide orientation, training, coaching, and mentoring to staff.
• Complete performance evaluations and handle disciplinary actions.
• Coordinate with operational leaders to scale and spread proven home care models
• Adjusts program plan as resource requirements change.
• Provide orientation, training, coaching, and mentoring to staff.
• Completes performance evaluations and handles disciplinary actions.
• Liaise between the Ascension Nursing Center of Excellence and Ascension sites of care.
• Align with research teams to study models of care in the home and helps identify key
  research questions surrounding model measurements.
• Work with the Ascension Nursing Center of Excellence Voice & Connection team to
  tell the story across diverse stakeholder groups.

What You Will Need
Licensure / Certification / Registration:

• Project Mgmt Professional preferred.

Education:

• High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
• Master's degree preferred.

Work Experience:

• 5 years of experience required.
• 10 years of experience preferred.
• 2 years of leadership or management experience required.
• 5 years of leadership or management experience preferred.

Additional Preferences
Additional Preferences:

• RN- acute care background high preferred
• Case management experience
• Comfortable publishing and presenting to key stake holders
• Home health knowledge/background
• Technology savvy
