As a remote LPN/LVN, you will manage patient engagement, document health status, and support mobility programs, maintaining patient confidentiality.

The role involves enrolling patients in Remote Therapeutic Monitoring and Chronic Care Management programs, documenting health status, and providing support through communication and health pointers.
