Lead employee relations, performance management, workforce planning, compliance, and retention initiatives for field-based clinical and non-clinical teams, ensuring multi-state legal compliance and support for high-acuity patient populations.

The Lead Care Manager serves as the primary client contact, coordinates care across providers, develops and updates care plans, connects clients to community supports, conducts outreach and documentation in EMR/CRM, uses evidence-based practices, and ensures program compliance and reporting.

Provide remote and field-based care management for vulnerable members: assess needs, develop and update care plans, coordinate providers and community resources, conduct outreach and engagement, document in EMR/CRM, monitor outcomes, and promote mental health and recovery using evidence-based practices like Motivational Interviewing and Trauma-Informed Care.
