Manages an Enhanced Care Management team of 10–15 staff, overseeing client outreach, enrollment, individualized care plans, care coordination, clinical sign-offs, and complex case escalations. Provides clinical oversight, leads interdisciplinary care meetings, coaches employees, monitors KPIs, supports process improvements, and delivers direct care management as needed. The role is remote, requires an active California LCSW or LMFT license, and includes up to 25% travel.

Designs, delivers, and evaluates training for Enhanced Care Management staff. Responsibilities include onboarding, compliance and continuing education, training needs analysis, curriculum and job-aid development, training records, audits, quality improvement, stakeholder collaboration, and alignment with CalAIM and Medi-Cal requirements. The role also monitors policy changes and supports cross-functional training projects.

Lead Care Manager coordinates person-centered care for clients with complex needs. Responsibilities include assessing physical, behavioral, and social needs; developing care plans and SMART goals; connecting members with healthcare, social, housing, transportation, and community services; advocating with providers; conducting outreach; documenting activities in CRM and EMR systems; tracking outcomes; and completing compliance reporting.

Partner with clinical leaders to align people strategies with operational goals across a multi-state clinical workforce. Lead employee relations, performance management, workforce planning, compliance, and retention initiatives for field-based clinical and non-clinical teams, ensuring multi-state legal compliance and support for high-acuity patient populations.

Lead design and delivery of scalable, cloud-native healthcare systems and real-time data pipelines. Architect event-driven, batch/stream hybrid solutions, integrate clinical data standards, optimize performance and costs, mentor engineers, and collaborate with product and stakeholders to deploy resilient, compliant services.
