Provide direct patient care under RN supervision in a home health setting: implement care plans, document care, educate patients/families, coordinate services with other disciplines, use equipment safely, and report concerns to RN/manager.

Serve as the community liaison to develop referral relationships and drive admissions for home health/hospice. Execute sales calls, territory planning, account development, in‑service education, CRM pipeline management, and budgeted marketing activities while ensuring regulatory compliance and adherence to company code of conduct.
