Provides comprehensive telehealth diagnosis and treatment for frail, elderly, and complex patients in their homes alongside field paramedics. Develops treatment plans, assesses readmission risks, educates patients and families, coordinates care with multidisciplinary teams, documents visits, and supports referring providers. Requires availability for 8–12-hour shifts, including one weekend day monthly, and proficiency with telehealth technology and electronic medical records.
