Provide skilled occupational therapy services in patients' homes per an OT-established plan of care. Instruct patients and caregivers, fit/adjust adaptive devices, document progress in EMR, coordinate with physicians and clinical team, meet productivity targets (27-32 visits/week), participate in discharge planning and on-call rotation, maintain licensure, certifications, and required immunizations.

Manage strategic PBM client relationships, lead contract renewals, present performance and cost analyses, oversee benefit design and implementation, mentor client services teams, and drive client retention and growth while ensuring compliance and timely issue resolution.

Manage provider network contracting and unit cost budgets, set targets, build geographically competitive networks, evaluate and negotiate provider contracts, ensure appropriate specialty distribution, lead a team, and contribute to financial forecasting and pricing models to manage medical cost and trends.

Lead enterprise technical product and program initiatives across a matrixed healthcare organization. Translate complex operational and risk data into executive insights, drive large-scale transformation, implement AI and automation, manage governance and stakeholder communication, and support CIO-level planning and executive operating rhythms.

Provide in-home personal care and assistance with daily living (bathing, dressing, grooming, ambulation), medication reminders, basic vital sign monitoring and documentation (using EVV), household support and meal prep, accompany clients to appointments, ensure safe environment and proper use of medical equipment, and respond promptly to client needs while maintaining confidentiality and professional boundaries.
