Coordinates care for high-risk patients with chronic conditions across acute, post-acute, and home settings. Conducts comprehensive physical, mental, and social assessments; manages transitions of care, medication reconciliation, referrals, discharge planning, and readmission reduction. Collaborates with physicians, multidisciplinary teams, patients, caregivers, payers, and community providers. Documents care in the EMR and uses clinical, fiscal, and satisfaction data to support quality improvement.

Sources, screens, interviews, and recommends candidates for healthcare roles while managing recruiting strategies, job postings, applicant tracking, offers, background checks, and onboarding. Partners with leaders to define hiring needs, builds diverse candidate pipelines, represents the organization at recruiting events, maintains recruiting reports, and ensures compliance with employment regulations. The role requires bilingual English-Spanish capability preferred, strong communication, independent judgment, and travel to centers and events.
