Case Manager (Care Coordinator)

The Source is seeking a compassionate, organized, and mission-driven Case Manager to walk alongside women and families by providing care coordination, support, and access to life-changing resources. What You'll Do
As a Case Manager, you will:
- Conduct client needs assessments and develop individualized Care Plans
- Connect clients with community resources and government assistance programs
- Provide ongoing encouragement, coaching, and care coordination
- Conduct follow-up calls, home and hospital visits
- Maintain accurate client documentation using Salesforce
- Lead and support client groups while collaborating with clinic staff to ensure excellent client care
What We're Looking For
Required Qualifications
- Committed Christian with a personal relationship with Jesus Christ
- Passion for serving women, children, and families while upholding the sanctity of human life
- Bachelor's degree in a related social services field, Registered Nurse (RN), or Certified Community Health Worker
- Strong organizational, communication, and computer skills
- Ability to manage confidential information with professionalism
Preferred Qualifications
- Case management or care coordination experience
- Experience in coaching, mentoring, or motivational interviewing
- Master's Degree in Social Work (Texas Licensed)
