SCOPE OF ROLE

Bright Health’s Community Health team is responsible for partnering with internal teams and external, market-based resources to ensure that members have both their clinical and social needs met. As part of Bright Health’s promise to provide quality care that is easy, simple and affordable, Community Health Workers lead community outreach, clinical and social coordination, and place our members on the path better health and a better life.  Community Health Workers are the organization and member’s primary contact for community engagement, implementing social determinants of health outreach programs aimed at improving member health outcomes, promoting trust and engagement with Bright Health and the health care system, and driving down costs by focusing on provision of social resources and connection to primary care.

ROLE RESPONSIBILITIES

The Community Health Worker job description is intended to point out major responsibilities within the role, but it is not limited to these items.

  • Member Engagement
    • Linking people to healthcare and social support resources.
    • Providing support and follow up on healthcare and social support needs.
    • Making in-person visits to members to facilitate healthcare and social support needs.
    • Helping individuals and families access resources, including social support, health insurance benefits, quality care, and health information
    • Management of a caseload of members, with includes outreach using a variety of mediums (phone, Skype, in-person) to update their progress, answer questions, make personalized recommendations and changes to their programs if necessary.
    • Build trusting relationships with members and connect them to care, eliminating barriers, and advocating for systemic changes along the way.
    • Create connections between members and the health care system.
    • Assist members with enrollment in programs and benefits for which they are eligible.
    • Educates member on the proper use of the Emergency Room and provide information for alternatives.
    • Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
    • Continuously expands knowledge and understanding of community resources and services. Facilitates member access to community resources and relevant mental health services. Assists member in utilizing community services, including scheduling appointments.
    • Travels extensively to member homes, community locations, various agencies, and other outreach destinations.
  • Administrative
    • Complete administrative tasks on an ongoing basis to ensure proper and organized record keeping, working with staff members when/if necessary.
  • Team Coordination
    • Works collaboratively and effectively within a team. Establishes positive, supportive relationships and provides feedback to other members of the team. Builds and maintains positive working relationships with members, providers, nurse case managers.
    • Other duties and responsibilities as assigned.

EDUCATION, TRAINING, AND PROFESSIONAL EXPERIENCE

  • Social Worker, Medical Assistant, Engagement Specialist, or Community Health Worker Certification; equivalent work experience and education will be considered in lieu of a certification.
  • High School Diploma or GED required; Bachelor’s Degree preferred.
  • Three (3) or more years of experience in the healthcare industry navigating relationships around providers, patients, community resources, and insurance networks is preferred.

PROFESSIONAL COMPETENCIES

  • Bilingual in Spanish and English required.
  • Previous health care experience, preferably in one of more of the following areas: community health, social services, population health, provider systems or health plans.
  • Excellent verbal and written communication skills.
  • Strong customer services skills.
  • Strong interpersonal skills, service coordination skills, and advocacy skills.
  • Ability to perform multiple tasks and prioritize simultaneously.
  • Skill in organizing resources and establishing priorities.
  • Experience working with individuals with social needs and/or with chronic health needs.
  • Experience using a CRM tool and/or demonstrated experience in documentation.
  • Demonstrated ability to create trusting relationships with people and comprehend the challenges they face.
  • Knowledge of local community agencies, resources and insurance benefits. Preference given to candidates with a strong connection to their community and resources available.
  • Demonstrated success in working as part of a multi-disciplinary team including speaking and working with Physicians, Registered Nurses, and other customer service representatives.
  • Ability to travel to various locations locally.
  • Proficiency with the Microsoft Office suite.

WORK ENVIRONMENT

Most work responsibilities are performed in an open office setting, carrying out detailed work sitting at a desk/table and working on the computer. Some local travel will be required.
