What You Will Do
As the Director of the Central Care Management Team, you'll have oversight for the Central CM Team.
Key Responsibilities:

  • Oversight and Management of the Central CM team.
  • Accountable for the Central Team obtaining NCQA accreditation for CM programs.
  • Steer operational process and program improvement that will drive process/model/performance improvement for the Central Team.
  • Provide oversight of clinical team activities, staff development, budget management and regulatory compliance.
  • Deployment and management for Clinical Operations for programs touching National Insurance Contracts:
    • SmartHealth, Ascension Complete, and Ascension Personalized Care.
  • Model and cultivate an internal cultural structure designed around collaboration, support, feedback, motivation, and accountability


Key Requirements of role:

  • Yearly this role will require education specific to cultural competencies and social determinants of health.
  • Yearly this role will review and complete the Clinical Competencies set forth in the National CM Program.


Direct the development and implementation of policies, procedures and strategies.

  • Organize operations to provide high quality, cost effective services.
  • Model leadership and teaming excellence by providing appropriate advice and counsel to staff to improve effectiveness.
  • Develop and implement policies and procedures that support the delivery of services to meet all productivity plans.
  • Coordinate and integrate services to meet contractual requirements.
  • Conduct ongoing assessments, maintain quality control programs, and improve departmental operations.
  • Direct staff. Manage departmental turnover while maintaining high employee and patient satisfaction.
  • Identify marketing and collaborative opportunities for the department.


What You Will Need
Licensure / Certification / Registration:

  • BLS Provider preferred.
  • Registered Nurse preferred. Licensure required relevant to state in which work is performed.


Education:

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
  • Master's degree preferred.


Work Experience:

  • 5 years of experience required.
  • 10 years of experience preferred.
  • 2 years of leadership or management experience required.
  • 5 years of leadership or management experience preferred.


Additional Preferences
Additional Preferences:

  • Strong understanding of NCQA, preferred.
  • Background in Health System and Health Plan Care Management, preferred
  • 3+ years of Care Management experience, perferred.
