Provide restorative and personal care to skilled nursing facility patients, assisting with ambulation, range of motion, positioning, self-feeding and other restorative nursing tasks. Support independence, report patient status to the Director of Rehab, contribute to care plans, document observations, and travel between nearby SNF locations as needed.
Location: Park Valley & Falcon Ridge OR Bel-Air & San Gabriel Rehabilitation
The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.
- Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.
- Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
- Promotes and supports the greatest possible degree of independence for patients.
- Advises Director of Rehab (DOR) of Patient's status daily.
- Contributes knowledge of Patient's conditions/needs to patient care plans.
- Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
- Observes and reports on Patients' conditions and documents observations as required.
- State certification as a Nursing Assistant is mandatory.
- Ability to read, write, and speak the English language.
- Must be available to work six days per week
- Ability to travel to additional SNF locations within geographical area
