As the Home and Community Bases Services Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.

• Full-time position providing in-home visits for hospice patients.
• Current Ohio STNA certification is required.
• Standard business hours are Monday-Friday, 8:00 AM-4:30 PM.
• The agency serves a 50-mile radius, and candidates must be willing and able to travel throughout the coverage area to provide patient care.

Primary Responsibilities:

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualification:

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation
• Must have successfully completed an approved Ohio Department of Health State Tested Nurse Aide (STNA) training program and competency, OR meet the qualifications to be Medicare Certified Home Health Aide, OR have at least one (1) year supervised employment experience as a Home Health Aide or Nurse Aide and has successfully completed a competency evaluation, OR has successfully completed a Council on Aging Learning Advantages (COALA) Home Health training program, a certified vocational or other sixty (60) hour training and competency evaluation program that meets the requirements listed in rule 173-39- 02.11(C)(3)(a)(v)(b).

Preferred Qualifications:

• Current CPR certification or ability to complete within 90 days of hire
• 6+ months of home care experience
• Ability to work flexible hours and independently

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.75 to $25.10 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
