Essential Job Functions:
Clinical Pathway Design & Operationalization
- Partner with the Senior Director of Clinical Strategy, physician leaders, and clinical stakeholders to design and operationalize clinical pathways across agilon’s enterprise portfolio, including COPD, CHF, Dementia, Preventative Health Planning (PHP), and future pathways.
- Translate clinical pathway strategy into end-to-end operational workflows, including provider, care team, data, and technology-enabled processes.
- Apply service design and process improvement principles to identify friction in care delivery and design solutions that improve clinical quality, efficiency, and patient experience for senior patients.
- Map and analyze current-state and future-state workflows to identify gaps, inefficiencies, and opportunities for standardization and scale across pathways.
Implementation & Scaling
- Lead the implementation and scaling of multiple clinical pathways across agilon markets, ensuring alignment with value-based performance goals, operational realities, and market needs.
- Develop and execute pathway implementation plans, including success metrics, timelines, and change management approaches.
- Partner with market leadership, physician groups, and care teams to support pathway adoption, sustainability, and consistent execution across diverse market environments.
- Ensure clinical workflow designs are supported by appropriate technology, data, and operational infrastructure.
Cross-Functional Collaboration
- Work in a highly matrixed environment across clinical, operational, product, data science, technology, legal, and compliance teams.
- Facilitate alignment between clinical strategy, product development, and operations to ensure pathways are feasible, scalable, and value-generating.
- Serve as a bridge between clinical strategy and frontline clinical execution, ensuring pathway intent is preserved through implementation.
Performance Measurement & Continuous Improvement
- Establish performance objectives and define outcome measurement frameworks for clinical pathways tied to quality outcomes, patient experience, total cost of care, and value-based performance.
- Monitor pathway performance post-implementation and drive iterative improvements based on data, feedback, and observed outcomes.
- Leverage standard process improvement methodologies (e.g., direct observation, process mapping, root cause analysis) to improve reliability and effectiveness of pathway execution.
Value-Based Care Enablement
- Support the ongoing evolution of agilon’s value-based care model by embedding clinical pathways into scalable, repeatable operational processes.
- Collaborate with colleagues across the organization to advance clinical programs that improve chronic and preventative care outcomes while supporting total cost of care management.

Minimum Experience
- 8 + years’ experience leading projects or driving initiatives
- Bachelor’s degree required; Master’s degree preferred

- Independent thinker who can manage ambiguity and devise solutions when a clear path otherwise does not exist while also being able to function effectively within team based environments.
- Proven skills in the development, implementation and execution of project plans, and the integration of complex operations.
- Advanced skills in Microsoft Excel to manipulate data and analyze large data sets
